Add comprehensive documentation for the fabric-orch CLI tool covering:
- Installation and quick start instructions
- Environment variables for gNMI authentication
- All discover subcommands (capabilities, get, set, subscribe, paths)
- YANG path syntax reference and common paths table
- Output formats and error handling guidance
